Ортопедическая реабилитация пациентов после тотальной резекции верхней челюсти остается одной из наиболее сложных задач челюстно-лицевой ортопедии вследствие обширных послеоперационных дефектов, нарушения сообщения между полостью рта и полостью носа, а также недостаточной фиксации протезов-обтураторов.</p></sec><sec><title>Цель</title><p>Цель. Разработать и оценить клиническую эффективность способа улучшения фиксации резекционного протеза-обтуратора при тотальной резекции верхней челюсти.</p></sec><sec><title> Материалы и методы</title><p> Материалы и методы. Проведена серия клинических наблюдений, включившая 16 пациентов с послеоперационными дефектами верхней челюсти после тотальной резекции. Предложен способ фиксации, основанный на формировании трапециевидного углубления на вестибулярной поверхности обтуратора с последующим размещением медицинской поролоновой губки или стоматологического ватного валика в качестве дополнительного ретенционного элемента.</p></sec><sec><title>Результаты</title><p>Результаты. Применение разработанного способа обеспечило удовлетворительную фиксацию и устойчивость протеза-обтуратора, надежное разобщение полости рта и полости носа, улучшение функций жевания, глотания и речи. Представленные клинические наблюдения подтвердили практическую применимость разработанной конструкции.</p></sec><sec><title> Выводы</title><p> Выводы. Предложенный способ является простым, доступным и эффективным методом повышения ретенции резекционного протеза-обтуратора и может быть рекомендован для ортопедической реабилитации пациентов после тотальной резекции верхней челюсти.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Prosthetic rehabilitation of patients after total maxillectomy remains one of the most challenging problems in maxillofacial prosthodontics because of extensive postoperative defects, communication between the oral and nasal cavities, and insufficient retention of obturator prostheses.</p></sec><sec><title>Objective</title><p>Objective. To develop and evaluate the clinical effectiveness of a technique for improving the retention of resection obturator prostheses following total maxillectomy.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. Two clinical cases of prosthetic rehabilitation after total maxillectomy using the proposed obturator retention technique are presented. The technique involves creating a trapezoidal recess on the vestibular surface of the obturator and placing a medical foam insert or a dental cotton roll as an additional retentive element.</p></sec><sec><title>Results</title><p>Results. The proposed technique provided satisfactory retention and stability of the obturator prosthesis, reliable separation of the oral and nasal cavities, and improvement of mastication, swallowing, and speech. The presented clinical cases demonstrated the practical applicability of the proposed design.</p></sec><sec><title>Conclusions</title><p>Conclusions.
