Буллезный эпидермолиз – это редкий генодерматоз. Поход к врачу для них проблематичен, так как передвижение в транспорте, переодевание травмирует их кожу.В отдаленно расположенных от столицы республики населенных пунктов Дагестана не хватает специалистов с соответствующим уровнем компетенций.Есть угроза инфицирования коронавирусной инфекцией при личном визите к врачу. Все эти проблемы телеконсультация снимает.Цель – адаптация телемедицинских технологий дистанционному формату верификации клинического диагноза и динамического наблюдения больных ВБЭ.Материалы и методы. В дистанционном формате обследованы 93 пациента с ВБЭ. Проводилось консультирование как врача, так и пациента. Динамическое наблюдение за частью из них (восемь человек) проводилось затем в дистанционном формате, использующем мобильное приложение для смартфонов больных.Результаты и заключение. С помощью телемедицинских технологий верифицирован развёрнутый клинический диагноз у 98% больных, и лишь у одного диагноз не уточнён. Повторные онлайн беседы были необходимы в плане уточнения имеющейся информации: сопутствующие заболевания, уточнения даты манифестации заболевания, наличие больных родственников с подобным диагнозом. Очно и дистанционно консультированные пациенты (127 человек) вошли в республиканский реестр пациентов, страдающих ВБЭ. У большинства больных зафиксированы множественные сочетанные случаи осложнения органов и систем - 614 случаев у 127 больных. В рамках диспансерного наблюдения 8 пациентов наблюдались в дистанционном режиме. Они отметили такой формат, как приоритетный. Руководство ЛПУ должно согласовать вопрос оплаты теледерматологического формата работы врачам. Необходимо также соблюдать требования онлайн консультирования: устранить посторонние шумы, дать качественный свет, а также чёткие изображения участков поражения при асинхронном консультировании. обеспечить соблюдение режима освещения и тишины,четкость фотоизображений.

Congenital epidermolysis bullosa (VBE) refers to orphan diseases. Face-to-face consultation is stressful for them, since moving in a car, in a wheelchair, dressing and undressing, removing bandages is very painful and difficult for the patient. There are not enough specialists with the appropriate level of competence in the settlements of Dagestan located far from the capital of the republic.There is a threat of infection during a personal visit to a doctor in a pandemic of coronavirus infection. Teleconsultation removes all these problems.The aim is to adapt telemedicine technologies to the remote format of verification of clinical diagnosis and dynamic observation of patients with VBE.Materials and methods 93 patients with VBE were examined in a remote format. Both the doctor and the patient were consulted. Dynamic monitoring of some of them (eight people) was then carried out in a remote format using a mobile application for patients' smartphones.Results and conclusion. With the help of telemedicine technologies, a detailed clinical diagnosis was verified in 98% of patients, and only one diagnosis was unspecified. Repeated online conversations were necessary in terms of clarifying the available information: concomitant diseases, clarifying the date of manifestation of the disease, the presence of sick relatives with a similar diagnosis. Face-to-face and remotely consulted patients (127 people) were included in the republican register of patients suffering from VBE.
