По данным регистра Международного общества трансплантации сердца и лёгких, каждый год проводится около 5000 трансплантаций сердца. Выживаемость после трансплантации сердца находится на высоком уровне, в сравнении с естественным ходом терминальной сердечной недостаточности, в связи с этим, с каждым годом увеличивается трансплантационная активность. В данной статье представлен клинический случай успешной ортотопической биатриальной трансплантации сердца реципиенту с терминальной стадией сердечной недостаточности, особенностью являлось наличие гигантского левого предсердия, объем которого перед операцией составлял 1070 мл, индексированный объём 660 мл/м², размер по длинной оси 10,3 см. Гигантское левое предсердие является редкой патологией, которая характеризуется чрезмерной дилатацией полости. По литературным данным, частота встречаемости данной патологии составляет 0,3 %. Прогрессирующая дилатация левого предсердия была ассоциирована с наличием выраженной митральной регургитации и фибрилляцией предсердий в анамнезе. Описано предоперационное состояние пациента, особенности хирургического вмешательства и послеоперационного ведения пациента. Проведена динамическая оценка состояния пациента в течение года, включая мониторинг лабораторных показателей, инструментальных исследований и функциональных тестов.

According to the registry of the International Society of Heart and Lung Transplantation (ISHLT), approximately 5,000 heart transplantation are performed annually. Survival rates after heart transplantation are significantly higher compared to the natural course of terminal heart failure, as a result, transplantation activity has been increasing every year. The article represents a clinical case of successful orthotopic biatrial heart transplantation to a recipient with end-stage heart failure, the peculiarity was the presence of a giant left atrium, the volume of which before the operation was 1070 ml, indexed volume 660 ml/m², long axis size 10.3 cm. Giant left atrium is a rare pathology characterized by excessive dilation of the cavity. According to literature data, the incidence of this pathology is 0.3%. Progressive left atrial dilatation was associated with the severe mitral regurgitation and atrial fibrillation in anamnesis. Description of the preoperative condition of the patient, features of surgical intervention and postoperative management of the patient. A dynamic assessment of the patient's condition was carried out throughout the year, including monitoring of laboratory parameters, instrumental investigations and functional tests.
