ҰЙҚЫ БЕЗІНІҢ АЛКОГОЛЬСІЗ МАЙЛЫ АУРУЫНЫҢ ҚАУІП ФАКТОРЛАРЫ БАР ЕРЕСЕКТЕРДЕГІ КЛИНИКАЛЫҚ-МЕТАБОЛИКАЛЫҚ ЖӘНЕ ҚАБЫНУ КӨРСЕТКІШТЕРІМЕН ЕСЕПТЕЛГЕН МРТ МАЙ ИНДЕКСІНІҢ АССОЦИАЦИЯСЫ

Ұйқы безінде майдың жиналуы жас ұлғаюымен, семіздікпен және метаболикалық бұзылыстармен байланысты. Өзара түзетуден кейін жекелеген метаболикалық және қабыну көрсеткіштерінің үлесі анық емес.

Зерттеудің мақсаты: метаболикалық қауіп факторлары бар ересектерде ұйқы безінің есептік жартылай сандық МРТ-май индексінің клиникалық-антропометриялық, метаболикалық және қабыну көрсеткіштерімен байланысын бағалау.

Материалдар мен әдістер: Астанада 2025 жылғы 25 қазаннан 2026 жылғы 30 сәуірге дейін тексерілген 30–60 жастағы 150 ересектің проспективті жиналған деректеріне көлденең талдау жүргізілді. МРТ-май индексі екі жаңғырықты химиялық ығысу кескіндері бойынша есептелді; ол стандартталған MRI-PDFF-ке тең емес. Краскел–Уоллис критерийі, Спирмен корреляциясы, көппараметрлі гамма-регрессия және сезімталдық талдаулары қолданылды. Көптік салыстырулар Бенджамини–Хохберг әдісімен түзетілді.

Нәтижелері: индекстің медианасы 5,70 [2,60–9,83] % болды. Ең айқын оң корреляциялар жаспен (ρ=0,445), ДСИ-мен (ρ=0,426), HbA1c (ρ=0,400), HOMA-IR (ρ=0,385), интерлейкин-6 (ρ=0,368) және инсулинмен (ρ=0,354) анықталды; ЖТЛП-мен байланыс кері болды (ρ=−0,240). Толық гамма-модельде тек жаспен байланыс статистикалық мәнді болып қалды: бір стандартты ауытқуға шаққандағы орташа мәндер қатынасы 1,510 (95 % СА 1,254–1,818; p<0,001). Жыныспен, ДСИ-мен және интерлейкин-6-мен байланыстар тек қосымша сызықтық модельде статистикалық мәнді болды.

Қорытынды: түзетілмеген талдауда МРТ-май индексі бірқатар антропометриялық, метаболикалық және қабыну көрсеткіштерімен байланысты болды. Өзара түзетуден кейін жаспен байланыс ең тұрақты болды. Индекстің жартылай сандық сипаты және басқа байланыстардың модельге тәуелділігі нәтижелердің клиникалық түсіндірілуін шектейді.
